Territorial Medicine

The evolution of health care models is increasingly shifting the focus from hospital care alone to the continuous management of the patient in the area. Taking care of chronic and frail patients requires tools capable of monitoring health status over time, facilitating contact between citizens and specialists and coordinating the activities of health professionals even outside hospital facilities. Exprivia promotes a vision of Connected Health, in which the hospital and the territory operate in continuity through digital tools that enable new models of care.

Our offer is natively designed for proximity healthcare through:

  • Territorial care and management of Individual Care Plans (PAI).
  • Multidisciplinary teleconsultation and teleconsultation, with secure exchange of data and reports.
  • Multi-parameter telemonitoring of chronic and frail patients, with automatic management of notifications on any out-of-threshold parameters.
  • Integrated Home Care (ADI) digitized, with centralized planning and reporting.
  • Territorial Operations Centre (COT) for the coordination of health and social services available in the reference area (e.g. Hospice, Community Houses, Community Hospitals, RSAs, etc.).
  • Management of Intermediate Structures through a configurable model that integrates the typical functions of an Electronic Medical Record, both inpatient and outpatient, adapting to the organizational needs of Community Hospitals, Territorial Rehabilitation Units, Hospices and RSAs.

All territorial functions are integrated with the Electronic Health Record and with the regional systems, allowing uniform management of clinical-care, administrative and governance processes, in line with regional digitization strategies.

In this context, continuity, traceability and consistency of care pathways in the area are guaranteed, with telemedicine no longer configured as a separate service, but as a fully integrated component of the health information system.

eLifeCare: the telemedicine solution for home care

The eLifeCare telemedicine solution provides a set of functionalities and services for Televisit, Teleconsultation and Telemonitoring to support all operators involved in the care and monitoring of the patient at home. In addition, it optimizes the activities and costs of the care protocol for the dehospitalized patient by improving its quality and offering an effective response to the needs of the new Digital Health.

By ensuring the convergence of the needs of the patient with the need for cost containment of healthcare companies and hospitals, eLifeCare revolutionizes the approach to the care of the dehospitalized patient thanks to the provision of services and technological infrastructures necessary for the complete and integrated management of care services, available and usable from tablets and smartphones.

  • The monitoring dashboard makes it possible to constantly monitor the patient in chronic or fragile conditions and activate immediate assistance in the event of detection of an out-of-threshold variation in clinical parameters.
  • The patient can easily get in touch with the specialists indicated for their pathology and request, as needed and at any time, remote specialist visits and consultations.
  • Operators and specialist doctors can have access to the patient’s medical record at any time, modify therapies, make prescriptions and visits or draw up reports.
  • The activities of the health personnel working in the area can be coordinated and monitored through the platform, to which all the surveys carried out by home operators at the patient’s home are sent.
